You need at least one primary document — such as a PSA Birth Certificate, Philippine Passport, or UMID card. If you don't have any primary document, bring any four (4) supporting documents including items like a Voter's ID, PhilHealth MDR, baptismal certificate, or barangay clearance. In all cases, also bring one recent 2×2 photograph with a white background and plain clothes.
Senior citizens (60 years and older) may use their OSCA-issued Senior Citizen ID as one of the supporting documents.
