Why Use the PhilSys Requirements Checklist Generator?
One of the most common reasons Filipinos fail their PhilSys registration appointment is arriving with the wrong or incomplete documents. PSA's requirements differ depending on your age, citizenship status, and reason for applying — and a single missing document means a wasted trip and a rescheduled appointment.
This generator was built to eliminate that problem. Answer three questions and get a precise, personalized checklist — no sifting through PSA circulars, no outdated blog posts. Just your exact list, ready to print.
What Documents Do You Need for the Philippine National ID?
The Philippine Statistics Authority (PSA) requires applicants to present at least one primary document or a combination of secondary documents to prove identity, date of birth, and address. The exact list depends on three variables:
Adults (18 and above) may present IDs that minors typically do not have. Minors require a parent or guardian's presence and their own birth certificate as the primary anchor document. Infants and children under 5 have an additional simplified pathway under the Special Program for Newborns.
Filipino citizens use PSA-issued documents (birth certificate, UMID, passport) as primary proof. Resident foreigners with permanent residency must present their Alien Certificate of Registration Identity Card (ACR I-Card) plus additional documents proving their Philippine address and identity.
New registrants need identity and address documents. Correction applicants need to bring supporting documents proving the correct information (e.g., PSA birth certificate for name/birthdate corrections). Lost ID applicants need an affidavit of loss plus original registration proof.
PhilSys Requirements — Frequently Asked Questions
Everything you need to know about what documents to bring for your National ID registration in 2026.
For adult Filipino citizens, you need at least one primary document such as a PSA Birth Certificate, Philippine Passport, UMID, or Driver's License. If you have no primary document, you may present two secondary documents including a Barangay Certificate of Residency, school records, or voter's ID. You must also present one document proving your current address such as a utility bill, bank statement, or barangay certificate not older than 3 months.
For minors, the PSA Birth Certificate is the primary required document, accompanied by the parent or guardian's valid ID. The parent or guardian must be physically present at the Step 2 registration.Download Digital Id Copy.
Yes, it is possible to register even without a PSA Birth Certificate, but it requires more supporting documents. If you do not have a birth certificate, you may present a combination of secondary documents that together prove your identity and date of birth. Accepted alternatives include a Baptismal Certificate, a school Form 138 or Form 137, a marriage contract (for married individuals), or an employment records certificate. PSA staff will evaluate the combination of documents presented. It is strongly recommended to secure your PSA Birth Certificate first — you can order it at serbilis.psa.gov.ph.
For a name correction, you need to bring your original PhilSys Transaction Slip, a PSA Birth Certificate (showing the correct name), and any government ID that reflects the correct name. If your name was misspelled at registration, you must also bring a notarized Request for Correction form available at the PSA office. For birthday corrections, the PSA Birth Certificate is mandatory as the primary supporting document. For address updates, a recent utility bill, barangay certificate, or bank statement dated within 3 months is required.
To replace a lost Philippine National ID, you need:
- A notarized Affidavit of Loss — stating when, where, and how you lost your ID (use a template from our site)
- Your original PhilSys Transaction Slip from your registration day
- At least one valid government-issued ID (passport, driver's license, UMID, etc.)
- A payment receipt for the replacement fee (currently ₱200 for lost cards; first replacement is free if the card is defective)
Visit your nearest PSA CRSRS outlet with these documents. Processing for lost ID replacement typically takes 6–14 months.
Yes. PSA requires you to bring both the original document AND one photocopy for each document submitted. PSA staff will verify the original and keep the photocopy on file. It is best to prepare photocopies in advance (A4 size, clear and legible) to avoid delays at the registration site. Do not submit documents with expired validity — IDs must be valid on the date of registration, not just when they were issued.
Yes, but only resident foreign nationals with permanent residency status in the Philippines are eligible. You must present your ACR I-Card (Alien Certificate of Registration Identity Card) issued by the Bureau of Immigration as your primary document. Additional requirements include a valid passport, proof of Philippine address (utility bill or lease agreement), and your BI Certificate of Registration. Temporary visitors and tourists are not eligible for PhilSys registration.
Discrepancies — for example, your passport says "Maria" but your birth certificate says "Marie" — are one of the most common causes of registration delays. PSA staff will flag the discrepancy and may require you to submit additional documents or a Supplemental Affidavit explaining the variation. In some cases, you may be asked to correct the underlying document (e.g., file an Administrative Correction at the Local Civil Registry) before your PhilSys application can proceed. Always bring all documents that show your name and birthday so staff can assess the issue on the spot.
Initial registration is free of charge for all eligible applicants under Republic Act 11055. The PSA is funded by the national government to provide every Filipino with a National ID at no cost. However, fees apply in specific situations: replacing a lost card costs ₱200, and replacement due to damage costs ₱200 (unless the damage was due to a manufacturing defect, in which case replacement is free). There are no hidden fees or service charges at official PSA registration sites.
