If the Birth Certificate shows the correct name but the PhilID has a different name, it is a PSA encoding error — visit any PSA main branch with your Birth Certificate and incorrect PhilID and file a free Data Correction Request. If the Birth Certificate also has the wrong name, file a petition under RA 9048 at your Local Civil Registry Office first. Processing takes 2 to 4 months after the PSA correction is filed.
      Common reasons: (1) Registration is less than 8 weeks old — digital ID takes 4 to 8 weeks to activate. (2) App needs updating — check for eGovPH updates in the Play Store or App Store. (3) Poor internet connection during QR generation — requires live server connection. (4) TRN entered incorrectly — no spaces, dashes, or extra characters. If none of these apply and it has been more than 8 weeks, visit a PSA branch to check your biometric record status.
      To fix a PhilID QR code that does not scan: clean the card surface of fingerprints or smudges; ensure the scanner app or device is updated; hold the card flat at 10 to 15 cm distance under good lighting with no glare; try a different scanner app. If the card is physically cracked or delaminated, file for a free replacement at any PSA center. Use your digital PhilID on the eGovPH app as a backup — it has the same legal validity under RA 11055.
