Why this template tool exists
An Affidavit of Loss is required by the PSA before they will process a replacement National ID for a misplaced card — but there is no official PSA template publicly available. Most Filipinos end up paying a law office PHP 500–1,000 just to have a basic affidavit drafted, when the format is standard and the content is straightforward.
This tool generates the complete, properly-formatted affidavit for free. The template follows the format used by notary publics nationwide and accepted by the PSA. You still need to have it notarized (which costs PHP 150–300), but you no longer need to pay for document drafting.
The Authorization Letter template fills the same gap — many Filipinos are abroad, bedridden, or otherwise unable to personally claim their National ID, and a properly worded authorization letter is what their representative needs to act on their behalf.
